Equipment responsibility form: a template for team radios
Radio rental · fleet tracking
The equipment responsibility form records who received each radio, its condition, accessories and return deadline. For rental at an event, construction project or factory shutdown, this simple document avoids hunting for radios on the final day and disputes over who kept a missing battery.

What is an equipment responsibility form?
It is a declaration signed by the recipient of work equipment. The person confirms receipt of the item in a stated condition, use for the agreed purpose, adherence to care instructions and return of all items on time. For two-way radios, the form must identify the device unambiguously: serial or asset number, team label, battery and accessories.
The form is not intended to transfer losses to the employee. It organizes custody. When each radio has a named person responsible, teams take better care of equipment, returns are quick and any loss is noticed immediately rather than days later. This matters to the company renting radios: the contracting company, not the guard or installer holding the device, is responsible for the kit toward the rental provider.
In companies with an asset policy, the responsibility form is often the same one used for corporate laptops and mobile phones. Radios merit their own annex because they circulate much more: changing hands each shift, returning to charging bases and changing channels according to role.
Responsibility form or custody record?
Both record custody, but on different schedules. The responsibility form is individual and covers a long period, such as the duration of a construction project or contract. The custody record, widely used in private security, records collection and return each shift in a list or book. For events and shutdowns, the most efficient combination is both: a form for each team leader and a shift custody record for whoever receives a radio from them.
| Situation | Recommended document | Who signs | When it is closed |
|---|---|---|---|
| One- to three-day event with rotating staff | Shift custody record, in a list | Each team leader and whoever collects equipment | On return at each shift |
| Construction project or shutdown with a permanent team | Individual form | Each radio user | At the end of the project or when changing roles |
| 24-hour security with post changes | Custody record in the post logbook | The guard taking over the post | At service handover |
| Contractors (agency, construction contractor) | Contractor’s form and named list | Person appointed by the contractor | At the end of the contract |

Shift custody: the radio control spreadsheet
In an operation with shift changes, radio control fits in a spreadsheet with one row per device. It can be printed and attached to the charging base or kept in a form on the supervisor’s phone, provided it records the time and responsible person.
Physical numbering makes the spreadsheet reliable. Number the radio and battery with the same number (R-07 and B-07), use a label color for each team and keep accessories in identified bags. Then checking returns takes seconds: the number missing from the table is the radio missing from the spreadsheet.

- Minimum columns: radio number, battery number, accessory, checkout time, person collecting it, return time, person returning it and condition.
- One row per checkout: if the radio returns for charging mid-shift, open another row; do not overwrite.
- Signature or record: initials on paper or confirmation on the form with date and time.
- Separate backup: backup radios have their own row and are only issued with the supervisor’s authorization.
- Shift closing: the supervisor checks the count before releasing the team and records any outstanding items.

Radio delivery and return checklist
The handover determines the quality of control. A minute of testing avoids the complaint that the radio “arrived faulty.”
- On delivery: turn on, check the channel and team name, make a test call to control and check the volume.
- Battery: confirm the charge and note the number; on long shifts, plan and record replacement.
- Accessory: test the earpiece or lapel microphone before leaving; an accessory with a loose connection makes the radio silent in the middle of the operation.
- Visual condition: secure antenna, buttons, screen and clip; photograph the assembly if there are any marks.
- On return: check the serial number, battery and accessories, look for moisture, cracks and antenna issues, and note the time.
- Reported fault: set the radio aside with a “do not use” label and describe the symptom in one sentence.
When radios are rented, ask the rental company for the serial number list for each kit. It becomes the basis of your agreement and return. For events, the post radio rental for events: what to test before opening the gates complements this checklist with an on-site coverage test.
Loss, theft or damage: what to do in the first hours
- Report immediately: the person informs the supervisor and control; the sooner, the greater the chance of finding the radio.
- Try to locate it: retrace the route, call the radio on the channel and ask neighboring teams.
- Record: note the time, last location and who had the device in the agreement or custody log.
- Theft: file a police report and keep the number for the company and rental provider.
- Notify the rental company: if the radio is rented, provide the serial number; some digital systems can disable the device when the feature has been configured.
- Restore the role: issue a backup radio to the team and record the replacement.
Common mistakes in radio control
- Generic agreement: “I received a radio” identifies nothing; without a serial number there is no way to hold someone accountable or find it.
- Forgotten accessories: earpieces, clips and chargers disappear more often than radios because nobody records them.
- Signature at the end: collecting signatures only during teardown turns the agreement into a formality.
- Too much data: under the LGPD, request only what is necessary for control; name, role and team are usually enough.
- Charging base without an owner: chargers without a responsible person become the place where radios disappear.
In an two-way radio rental, ABC Mais Telecom programs radios by team and details each kit’s items in the proposal, making it easier to prepare the agreement and custody log from the first shift. Frequently asked questions about the equipment responsibility agreement
Does the equipment responsibility agreement have to be on paper?
Not necessarily. Many companies use electronic forms recording the date, time and signer’s identity. Law no. 14.063/2020 addresses electronic signatures; confirm with legal which type your policy accepts.
Can the company deduct from salary if the radio breaks?
Only in the cases under article 462 of the CLT: when the deduction was agreed or when there was intent. Even so, each case must be investigated. Define the rule in the internal policy and validate it with HR before including it in the agreement.
What is the difference between a responsibility agreement and a custody log?
A responsibility agreement is individual and covers a long period. A custody log records checkout and return for each shift in a list. At events and shutdowns, the two complement each other.
Does a rented radio also need an agreement?
Yes. The contracting company is responsible to the rental provider. The internal agreement assigns custody among people and makes return quick and free of outstanding items.
What should be recorded when a radio returns faulty?
The radio number, symptom in one sentence (does not transmit, static, does not charge), time and who was using it. Set the device aside with a “do not use” label and notify the rental company or technical assistance.
How long should agreements and custody logs be kept?
For the period defined in the company’s document policy. Under the LGPD, retain only the data necessary for control and only for as long as the purpose requires.
